New Student Intake Form

This form must be completed by all international students after they have been accepted to Rice University. Students requiring Rice’s visa sponsorship, on an F-1 or J-1 visa, must also submit supplemental documentation, including evidence of adequate financial resources to receive their immigration documents (Form I-20 or DS-2019).

Please complete this form and return it with the requested documentation to OISS via your OISS Portal account (http://oiss.rice.edu/oissportal). Be sure to also keep copies of all financial documents submitted, as you will need to present them to the U.S. consulate/embassy when applying for a visa and possibly to U.S. immigration officers upon arrival to the U.S.

If you have had F-1 or J-1 status previously, but are no longer eligible for a SEVIS transfer, please respond "No". In addition, if you have a current J-1 with an Exchange Visitor Category other than "Student", please respond "No", since your SEVIS record cannot be transferred from category to another.
If you will begin your program at Rice with an externally sponsored J-1 visa (e.g., Fulbright/LASPAU), please select "I have/will have a J-1 visa sponsored by Fulbright for my studies at Rice."
E.g. student, professional title, etc.

Please provide information about the institution that sponsors your current F-1 or J-1 visa.

Guidance on Summer Activities for Music Students

Many music students engage in summer activities before starting their program at Rice while already on Rice-sponsored F-1 visa status. Failure to comply with immigration regulations, including securing proper work authorization, can result in a termination of your F-1 visa status that would require an immediate departure from the U.S.

To ensure compliance, please read through the information provided at https://oiss.rice.edu/summermusic. By typing your full name below, you confirm that:
1) You have read and understood the information at https://oiss.rice.edu/summermusic about Summer Activities.
2) Your summer plans are purely recreational in nature and therefore would not adversely affect your immigration status.
3) If your plans change, you will notify Rice OISS immediately and before participating in any activities that may require work authorization.

Type your full legal name here to indicate that you have read and agree with the Music Student Summer Activities Statement of Understanding above.
