Before you initiate the F-1 Graduate Music Student Curricular Practical Training (CPT) Form, please read the following important information about form routing.

Please note that incorrectly routed forms that are not fully completed and signed by the appropriate participants will be rejected by OISS, and a new form must be completed. Therefore it is very important that you enter the participants very carefully in the correct fields when signing and submitting the form.

Once you have completed and signed the form, and entered the participant information, you will be asked to enter your own email address. You will then receive a message with a link to confirm your email address and signature. Please make sure to use your Rice email for this purpose!

When all applicable participants have signed the form, you will get an email with the completed form attached. You may then submit your CPT request to OISS at https://oiss.rice.edu/applyCPT. Please note that all CPT requests must be submitted to OISS at least one week before the requested start date.

Participant 1 – Student

This is you! Please complete the Student section (pages 1 and 2) of the form below. Please note that once you sign the form on page 2, you will need to click on "Click to Sign" at the bottom of the form. "Click to Sign" will only appear once all required questions have been answered.

Once you have clicked to sign, you will be asked to enter the information for the other participants, as indicated below.

Participant 2 – Course Instructor

For a CPT based on course enrollment, you must collect signatures from both your course instructor and faculty advisor. Sometimes it is the same person, in which case you will need to enter their information twice. Please enter your course instructor's information as Participant 2. If your CPT request is based on thesis/dissertation research, leave Participant 2 blank.

Participant 3 – Faculty Advisor

All CPT requests, regardless of curricular justification, must be signed by your faculty advisor. Please enter your faculty advisor's information as Participant 3.

Participant 4 – Dean's Office

If you have received 20 or more CPT authorizations during your degree program and/or 6 or more CPT authorizations during the semester this new request is for, you must secure additional authorization from the Dean's Office. The form will alert you if this requirement applies, when you complete the section for previous CPT information. If your number of previous CPT authorizations reaches the indicated threshold, you must enter the Dean's office contact information as Participant 4 (check with the Shepherd School to confirm the appropriate contact to use). Note: You must expand the participant list in order to add the final participant.
